Transition Checklist

(Use this checklist to help you keep track of what steps have taken place)

  1. My child has been referred to my LEA (at 2 years, 6 months of age).
  2. The Transition Planning Meeting has been held and a Transition Plan and a timeline of activities has been developed.
  3. All needed screenings and evaluations have been completed for my child.
  4. Eligibility for special education services has been determined for my child.
  5. I have helped gather copies of the following records and information for my LEA:
    • Birth Certificate
    • Proof of Residence
    • Physical Exam
    • Shot (Immunization) Record
    • Social Security Number
    • Medicaid Card (if applicable)
    • Emergency Contact (family members, doctor, special instructions in case of an emergency, etc.)
    • Infant-Toddler Program Records
    • Other Preschool Program Records
    • Hospital, Clinic, or Medical Records (if applicable)
  6. If my child is not eligible for special education services from my LEA, other choices have been discussed with my Child Service Coordinator and/or the Preschool IEP team.
  7. If my child is eligible, I have visited my LEA's preschool sites or other community program options for my child.
  8. I have talked with my child's current service providers about possible changes in services and/or providers.
  9. My child's IEP meeting has been held and goals for the coming year have been set (may be done 30 days before age 3).
  10. Placement for my child has been determined, based on my child's IEP.
  11. If needed, transportation has been arranged.
  12. I have met and talked with my child's new teacher(s) and therapist(s).
